Please fill in the following forms to submit your dossier for review.

A TATP staff member will be assigned to review your dossier. This staff member will contact you shortly to set up a schedule for your review and a follow-up meeting.

If the TATP staff member asks for revisions on your dossier, you must resubmit by an agreed upon date. Your dossier will not be accepted (with regards to the AUTP certificate requirements) until the TATP staff member reviewing your dossier approves it.
